PATIENTS WITH DENTAL INSURANCE
If you have DENTAL INSURANCE, please give us all the information so we can help you maximize your benefits. While we never base treatment needs or options on dental insurance, we do want to help you receive the appropriate coverage and benefits. O’BRIEN DENTAL is a BLUE CROSS / BLUE SHIELD PREFERRED PROVIDER.
As a courtesy, we will file all insurance claims for you at NO CHARGE. Please be sure you have all your insurance information available. (Insurance card, contract number, group number, name of insured if other than patient, insured’s social security number, date of birth, and employer or policy holder.)
Most insurance plans have deductibles, co-payments, and some excluded procedures. Payment for these amounts is expected at the time of treatment. If your insurance pays more than we expect, any overpayment will be credited to your account or issued as a refund upon request. Please note that any fees not covered by insurance for any reason will be the responsibility of the patient. Fees for treatments totaling $200.00 or less that are not covered IN FULL by insurance will be due at the time of treatment. If you have any questions or concerns about your coverage, please ask – we want to help and avoid any misunderstanding!
PATIENTS WITHOUT DENTAL INSURANCE
Patients without dental insurance or patients with insurance policies that will not assign payments to us will be expected to pay at the time of treatment. Please see our payment methods and financing options below.
PAYMENT METHOD OPTIONS
Our payment options include cash, check (with proper credentials),VISA, MASTERCARD, DISCOVER, AND AMERICAN EXPRESS credit cards as well as debit cards.
Financing options include 12 months no interest (for qualified patients) or longer term financing (interest accrues) through CARE CREDIT. For larger treatment cases, we offer 90 days same as cash with credit or debit card monthly draft, or monthly bank draft. Ask if your treatment plan qualifies for one of these financing options. Due to the high administrative costs, we do not have monthly billing.
